Dear Contestant,

We hope you are well and looking forward to being on Blockbusters.

We have enclosed a Contestant Release form for you to sign. Everyone who 'contributes' to a television programme has to sign one so please read through it carefully.

If you are under the age of 18 please ask a parent or guardian to also sign it for you - THIS SECTION MUST BE FILLED IN BEFORE YOU TAKE PART IN THE SHOW.

PLEASE REMEMBER TO BRING THE FILLED IN FORM WITH YOU TO THE
STUDIO.

We know a couple of people who did not receive their Contestant Information Letter & Pack — if you have not received yours please contact us as soon as possible as they have now all been sent out.

If you have any questions please do not hesitate to call us. We will be calling you to reconfirm the date nearer to your recordings. We look forward to seeing you at the Studio to play 'Blockbusters'.
